As you know, the Affordable Care Act (ACA) previously required applicable large employers to furnish all employees covered under their health plan, as well as non-covered eligible employees, with 1095-C forms each year. However, due to recent legislative changes, employers are no longer required to automatically distribute these forms. Therefore, beginning with the 2025 plan year, which impacts 1095-Cs distributed in 2026, Illinois Tech will furnish 1095-Cs only upon request. Please see the notice below (which is also available on Human Resources’ Benefits website), regarding how to opt-in to future notices.
Please note that all covered and non-covered eligible employees will receive a 1095-C form for 2024 in the mail soon. The notice below pertains to 1095-C forms that will be distributed in 2026 and subsequent years.

Paperwork Reduction Act Notice of Need to Request Form 1095C
The Paperwork Reduction Act amends the Affordable Care Act by no longer requiring employers and health insurance providers to send form 1095C to employees and/or covered individuals under a health plan. Previously employers and/or insurance providers had to send 1095 forms to each full-time employee and/or covered individual. Now, those forms must only be sent when affirmatively requested by an employee and/or covered individual. If an individual requests a form, the form will be provided by January 31 or 30 days after the date of the request, whichever is later. This Notice serves as the required Notice employers must provide to inform individuals of their right to request a 1095C form.
