Illinois Tech BCBS Health Plan members and their eligible dependents* have all the FREE support they need to manage hypertension and type 2 diabetes, as well as to help prevent type 2 diabetes, through Teledoc Health (formerly Livongo). Teledoc Health programs include personalized action plans, one-on-one coaching, smart devices, real time support, and more!

There are four programs available to you and your family members:

• Diabetes Management
• Diabetes Prevention
• Hypertension Management
• Weight Management

*Eligible dependents include spouses and dependent children ages 18 and over. Only individuals with applicable health conditions will qualify (e.g., individuals with blood pressure in the normal range will not qualify for the hypertension program).
