In an effort to facilitate our ability to plan for the fall semester, the university is interested in whether students, faculty, and staff have been vaccinated, plan to get vaccinated, or do not anticipate getting vaccinated. You will receive a follow-up email with the survey link to participate.

Please understand that this survey is voluntary, but we strongly encourage you to complete it. This information will be invaluable to university leaders as they work to plan for the future.

Because we are asking you to share information about your health, the only people who will have access to individual responses are Hilary Hudson Hosek and Viktoria Rill from the Office of Human Resources. All information will be maintained in accordance with applicable privacy laws. To this end, only aggregate data will be reported to senior leaders. No individual personal information or status will be shared.

Please know that the university may undertake another survey as we get closer to the fall semester and plans come into greater focus.
