Flu and bivalent COVID-19 booster shots for faculty and staff will be available Thursday, November 10, from 10 a.m. –noon, in the HR Training Room on the 13th floor of the Galvin Tower.
Registration for this event is required and closes on November 8! Please use this online scheduler/eConsent link to reserve your appointment. This link contains the electronic consent form each participant must complete to receive a flu shot. Walk-ins will ONLY be accepted if there are enough vaccines and supplies.
Only PPO insurance plans are accepted for this event, and participants MUST bring their ID cards. Individuals covered under the Illinois Tech health plan through BCBS should bring their CVS/Caremark ID card in addition to their BCBS ID card.
Those who are planning on receiving a COVID-19 bivalent booster should also bring their COVID-19 vaccine card.
