Healthcare reporting forms for 2019

Due to requirements under the Affordable Care Act, you may receive some forms this winter providing you with information about the health coverage you had or were offered in 2019. The most common forms are the 1095 series forms. These forms include information about coverage offered to you and any covered dependents, and must be sent to eligible individuals regardless as to whether coverage was elected or waived.

For benefits eligible Illinois Tech faculty and staff: 1095-Cs for those eligible for coverage under Illinois Tech’s employer-sponsored coverage (Blue Cross Blue Shield) were mailed the week ending January 31, 2020. This is well in advance of the IRS’ March 2 postmark deadline.

For Illinois Tech students: 1095-B forms for those eligible for coverage under the Aetna student health plan are available from the health plan directly. Students may view/print the tax form online through the member site under Messages, and then under the Letters and Communications tab.  Students may also call 1-855-531-6837 to request a form be mailed to them. Individuals who need a social security number updated, may update it online via the member site and will immediately see the corrected form.

As has been the case in prior years, these forms do not need to be filed with your taxes.  If your tax professional requires you to provide a copy and you do not receive yours by March 3, employees should contact Viktoria Rill at 567-3353 or vrill@iit.edu, and students should contact Student Health and Wellness at 567-7550.
