Your Immunization Verification Form or a copy of your immunization records is due on January 26, 2013. If you are unable to provide proof of immunization compliance, you are welcome to receiving the required vaccinations at the Student Health & Wellness Center (IIT Tower, 3rd Floor) during our Open Vaccine Days on January 18 & 25, 8:30 -11 am and 1 – 4 pm.  No appointment needed; just come on in!  If you purchased the IIT Health Insurance plan, the vaccinations are 100% covered and at no cost to you.

A class registration hold will be placed on any non-compliant student’s record on April 2, 2013. Please email us at student.health@iit.edu to inquire about your immunization status.
