The Student Health and Wellness Center (SHWC) reminds students that the deadline to submit an Immunization Verification form or begin a vaccine series must be submitted by Monday, September 22, 2014. Students that fail to submit their Immunization Verification form by the deadline will have a registration hold placed on their student account. In addition, the deadline to enroll or waive in the Student Health Insurance Plan (SHIP) has been extended through Monday, September 15, 2014. Application to waive the IIT SHIP can be made online here. Students who choose to enroll should request the form via email to student.health@iit.edu. Include the student ID number and date of birth. Any student that is full-time, on an F1 or J1 visa, or living in the residence halls, is automatically enrolled in the SHIP and will be billed $1,159. Any student that begins the waiver/enrollment process and has a record of it will not be penalized if they miss the September 15, 2014 deadline.
