Are you a faculty or staff member who is also a student at Illinois Tech? If so, you must complete the Aetna student health plan’s online waiver form in order to avoid being charged for student health coverage this fall.

The online form must be completed by September 1, 2020, and can be found here: https://www.universityhealthplans.com/letters/letter.cgi?group_id=436.
